The flag of Maryland Heights can be seen in a photograph on the city's website that shows the Maryland Heights flag flying alongside the American and Missouri flags. The flag itself features the circular city logo in three shades of blue centered on a white field. The logo shows the silhouettes of a house and skyscrapers in dark blue along a waterfront in light blue. The silhouette of a sailboat on the water is in dark and medium blue, with a
tree between the house and skyscrapers also in medium blue.

A fact worth noting is that the flag employs three shades of blue - there has been an increasing number of flags with two shades of blue or another color.
