The flag is a yellow field with the town seal in the center. This was confirmed to me by the town council via an email.

The seal shows a white ring with black and yellow outlines. Inscribed on the ring is the town name, along with the establishment and incorporation years, all of which are separated by yellow stars. In the center is an outline of the Choptank river. Above is a banner which states the motto "Bountiful jewel of the Choptank". Under the river is a scene which shows a boat passing under a bridge which connects two communities together. The River is the Warwick river, and the bridge connects the town of Secretary with an unnamed unincorporated community. At the bottom of the river are three types of marine wildlife. These are the clam, fish, and crab.
