It is a typical Portuguese communal flag, with the coat of arms centred on a plain yellow field.

This flag and arms replace a previous version, its only difference being the number of visible towers in the mural crown: before it was three . This change was enacted to reflect the attribution of town status to the named settlement.

Shield Azure, a chapel Or masoned Sable and with port and windows Sable and a bell tower, flanked by to maize ears Or leafed Argent, in chief a shuttle Or with yarns Gules in fess, on base wavy Argent a barrulet wavy Azure. Mural crown Argent with four visible towers (town rank) and white scroll reading in black upper case letters "VILARINHO - SANTO TIRSO".
