W. Smith in 1975 and also my sources reported a white over red
flag. However, this is incorrect, but it is possible that there
was a flag change. The red over yellow version was reported in
the 1980 Smith's book, but was marked as unsure.

The flag of the Government of Azuay can be seen on Twitter (archived). It is white with the logo of the government.
The logo depicts a coat of arms with the flag of Ecuador within it; below this are the words "GOBERNACIÓN DEL AZUAY"
